Vendor Registration Information

The individual or business who wishes to provide services to the ELC Broward must complete the forms below in order to complete the application process. You will be notified the forms that may or may not apply to you. In some cases, you may be asked to certify additional assurances; if such is the case, you will be notified. Each form must be completed in its entirety.

Once all forms are complete, please email scan and email to:  rfp@elcbroward.org

Please allow 7-10 business days for processing. 

If any of the information provided to ELC Broward should change, it is the vendor’s responsibility to notify our office by completing a new Vendor Registration Form along with any pertinent evidence of the change, as applicable.

The Early Learning Coalition of Broward County does not guarantee approval of the application. If your application is denied or is missing information you will be notified via email. If, after 10 calendar days, you have not submitted the information requested, your application is determined denied.
