How to Submit a School Readiness Provider Transfer
If you are currently receiving School Readiness services and would like to transfer your child/ren to another program, you have come to the right place. Please view the brief School Readiness Provider Transfer video below for instructions, then you will need fill out an ELC Transfer form to request that we open your Family Portal account. You will receive an email when your account has been opened, allowing you to request the transfer. Your account will be opened within one business day of your request.

If the parent/guardian is unable to access their family portal account the parent/guardian may send the completed form to the Family Services Department by email at firstname.lastname@example.org or bring the completed form for processing to the SR office listed below.
